Member Organization Agreement

Member Organization Agreement (Required)

This annual agreement is the required commitment of each member organization choosing to be affiliated with HFA.  It includes an overview of member benefits, a memorandum of understanding, your member organization information and your board selection. Copy of Form

Annual HFA Dues (Required)

HFA dues are a modest $1,000. By paying dues, you further demonstrate that being a part of the national HFA team has true value to your organization. Once we receive your member agreement, you will be mailed an invoice. Please note, if your organization is experiencing difficult financial times and you need special consideration, call HFA’s President/CEO to privately discuss.

Program Partner Agreement (Optional)

Each new year provides HFA the opportunity to partner and deliver our community based programming in exciting ways that offer flexibility for our member organizations. You will request the number of advocacy, programming or general HFA sessions you wish to provide locally. There are guidelines for delivery of HFA programs/services. A member of HFA’s Programming’s team is available to discuss how we can best support and partner.

Dateline Federation Agreement (Optional)

HFA’s national newsletter, Dateline Federation, currently distributes to over 15,500 individuals.

This publication is intended as a support tool for your local membership, reaching individuals with another outreach “touch” to the bleeding disorders community by mail 4 times per year. Articles are educational as well as providing local showcase stories about your membership and community. It is a free publication.

Member Organizations can choose to add this to your own outreach services and mail directly to your membership.  Your mailing options include:

1. Direct Mail: send Dateline directly to your membership (most cost effective)

With this option, you can work with our mailing house to coordinate your mailing list. A Confidentiality Agreement is in place and your mailing list is not provided to HFA or anyone else.  The mailing house will direct mail from this list. As an added benefit, the mailing house will verify this list with the USPS sorting tool (to verify that addresses are USPS deliverable addresses) and connect with you directly on any bad addresses so you can update your own system.

2. Bulk Mail: a quantity of Dateline Federation will be sent to your organization for you to direct mail *

If this is your choice, you will be responsible for addressing and mailing the Newsletter in a timely manner. Member Organizations may request reimbursement for the postage incurred to mail by sending an invoice directly to HFA.

* This duplicates mailing expenses and is not as cost effective, but we believe strongly in getting information out to our community and will work with you to make this happen.
